After Mastectomy: What Are Your Options?
A mastectomy marks the end of one chapter — and the beginning of decisions you may not have expected to face. This guide covers the options available to you for nipple and areola restoration in Sydney, including what 3D areola tattooing is, who it's for, and when it might be right for you.
Whether you've recently had a mastectomy or you're further along in your recovery, you may be wondering what comes next — particularly when it comes to the appearance of your chest. The good news is that you have more options than many people realise, and the decisions can be made entirely on your own timeline.
This guide is written for breast cancer survivors, people who have undergone prophylactic mastectomy, patients working through breast reconstruction, and the nurses and surgeons who support them. It covers the main nipple and areola reconstruction options available in Sydney — including the growing role of 3D areola and nipple tattooing as a final, meaningful step.
First: you don't have to decide anything right away
One of the most important things to understand is that there is no timeline you need to follow. Some women choose to have nipple-areola reconstruction as part of their breast reconstruction process. Others wait months or years. Some decide it isn't something they want at all — and that is equally valid.
If you are still in active treatment, managing fatigue, or simply not ready to think about this yet — that is entirely okay. This information will be here when you need it.
"There is no right timeline. The most important thing is that when you are ready, you know what's available to you."
What are the main options after mastectomy?
When it comes to nipple and areola restoration after mastectomy, there are three main paths. They are not mutually exclusive — many people combine them.
Surgical nipple reconstruction
A plastic surgeon creates a raised nipple mound from local skin tissue. This is done as a separate procedure, usually at least 3–6 months after the final breast reconstruction. The result is a physical projection — though sensation is typically limited. Areola tattooing is often used after this step to add colour and realism.
Prosthetic nipples
Silicone prosthetic nipples are custom-made and adhere to the skin. They can look very natural and are removable, making them a flexible option. Some people use them temporarily while deciding on a longer-term solution; others prefer them permanently. They require daily application and care.
3D areola and nipple tattooing
A specialised cosmetic tattoo technique that uses shading, highlight and colour to create a hyper-realistic nipple-areola complex on the skin. It can be performed with or without prior surgical nipple reconstruction. Results are long-lasting, non-surgical, and — when done well — remarkably natural. This is the focus at Aesthetic By Reese.
Why many patients choose 3D areola tattooing in Sydney
Over the past decade, 3D areola tattooing has become the most widely chosen option for nipple-areola restoration after mastectomy. It's not hard to understand why:
- No surgery required — no anaesthetic, no recovery ward, no weeks of healing
- Highly realistic results — the 3D shading technique creates the appearance of depth and projection that flat skin alone cannot
- Minimal downtime — most patients return to normal activity within days
- Flexible timing — can be done after reconstruction, after surgical nipple creation, or as a standalone procedure
- Long-lasting — results typically last several years before a refresh is needed
- Can be done on one or both sides — perfect for unilateral mastectomy where symmetry matters

Book a ConsultationUnderstanding your breast reconstruction timeline
If you've had or are planning breast reconstruction, the timing of areola tattooing depends on which type of reconstruction you've had:
Implant-based reconstruction
Whether immediate or delayed, implant reconstruction is one of the most common approaches in Australia. Areola tattooing is typically recommended at least 6 months after the final implant procedure — once the breast has settled into its final position and any scarring has matured.
Flap reconstruction (DIEP, TRAM, latissimus dorsi)
Flap-based reconstruction uses your own tissue to create the breast mound. These are more complex surgeries with longer initial healing periods. Areola tattooing is generally advised at least 6–12 months post-surgery. The tissue typically takes pigment beautifully.
Mastectomy without reconstruction
Some people choose not to have breast reconstruction — and that is a completely valid choice. Areola tattooing can still be an option, depending on the scarring and skin condition. A consultation will determine what is possible and what will look best for your individual situation.
After radiation therapy
Radiated skin behaves differently — it can be more fragile, less elastic, and slower to heal. As a general guide, areola tattooing is recommended at least 12 months after completing radiation. A patch test is strongly advised to assess how the skin takes and holds pigment. This is always discussed thoroughly at consultation.
- After mastectomy (no reconstruction): 4–6 months post-surgery
- After implant reconstruction: minimum 6 months after final procedure
- After flap reconstruction: 6–12 months post-surgery
- After surgical nipple reconstruction: 6–8 weeks after nipple has healed
- After radiation: minimum 12 months post-treatment
These are general guidelines only. Your surgeon's clearance is always recommended before proceeding, and individual circumstances may vary.
What does the areola tattoo procedure actually involve?
The process begins with a private, unhurried consultation. This is where everything is discussed — your medical history, your reconstruction, your goals, and what the result will look like. Colour matching is done carefully, using your natural complexion and existing breast tissue as reference.
On the day of treatment, a topical numbing cream is applied and given time to work. The tattoo itself builds in layers — creating shadow, midtone and highlight to simulate the three-dimensional appearance of a raised nipple. The procedure typically takes 1.5 to 2.5 hours depending on complexity.
A follow-up appointment at 6–8 weeks is standard — this is when colour is assessed, any fading adjusted, and the final result locked in.
Can I get areola tattooing on only one side?
Yes — and this is very common for unilateral mastectomy patients. The goal is to create symmetry with the natural breast, so the size, colour and placement of the tattoo are matched carefully to your natural side. The result can be remarkably balanced.
What about the cost? Is it covered by insurance?
Some Australian private health funds offer partial rebates for cosmetic tattooing following mastectomy — it is worth contacting your health fund directly to check your entitlements and whether a referral supports your claim. Medicare does not currently cover cosmetic tattooing.

Frequently asked questions
Do I need a referral to book a consultation?
No referral is needed. You are welcome to book directly. If you are working with a breast care nurse, surgeon or oncology team, they are welcome to be involved — but it is not a requirement to make an appointment.
I haven't had reconstruction. Can I still get areola tattooing?
Possibly — it depends on the condition of the skin and scarring. A consultation will determine what is achievable. Some patients who have chosen a flat closure are very happy with areola tattooing as a way to restore a sense of wholeness without reconstruction.
Will it look real?
When performed by a specialist, 3D areola tattooing can look genuinely natural — not "drawn on", but present. The shading technique creates the illusion of depth and dimension. The gallery on our service page shows real results from real patients, shared with their permission.
How many sessions does it take?
Most patients achieve their result in two appointments — the initial session and a follow-up at 6–8 weeks. Some cases, particularly where previous scarring or radiation has affected the skin, may benefit from an additional session.
Can I bring someone with me?
Absolutely. You are welcome to bring a support person to any appointment. The space is private and designed to feel calm and unhurried.
